Past forward: Generational perspectives on trans and non-binary inclusive medical education, from history-anchored teaching to practice change

The Project

This project explores how different generations within medicine understand and respond to trans and non-binary people, and how this affects care. Public debate is often polarised, and this can create uncertainty and anxiety for staff, with real consequences for patients. The project aims to support all clinicians to provide safe, fair and evidence-based care.

The project has four linked strands. Strand 1 uses generational theory as a framework for the study. We will examine what we mean by generational differences, what shapes them, and how experiences of medical school, clinical training, life histories, and changing relationships with healthcare influence views on trans and non-binary issues and on equality, diversity and inclusion more broadly.

Strand 2 focuses on the experiences of trans and non-binary medical students and doctors. Through in-depth interviews, we will explore what support and inclusion look like in practice, what helps people feel safe and valued, and how generational differences shape these needs. This strand also links to broader questions about public attitudes. We will review existing survey data and, where gaps remain, design new survey work with trans communities to understand wider public attitudes.

Strand 3 takes a historical approach. Working with historians, the project will uncover and share stories of trans and non-binary people, past and present, through a public website, an exhibition, and social media. This will show that gender diversity is not new and will broaden understanding beyond a narrow focus on medical transition.

Strand 4 brings everything together for medical education. Using insights from the earlier strands, this work will co-produce and evaluate training resources that address generational gaps, integrate historical perspectives, and centre lived experience. This includes resources on contested areas such as trans participation in sport, aimed at breaking down assumptions through evidence rather than politics. All materials will be reviewed with trans and non-binary participants to ensure they are accurate and useful.
